Social Science Program Specialist




This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center’s Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. The position provides care to Veterans seeking self-directed, lasting changes aligned with their values.

The incumbent will serve in a developmental assignment to provide the individual with the knowledge, skills, and abilities needed to perform the full range of duties as described in the targeted position description. Health Coach duties include:

  • Contacts Veterans for enrollment into Whole Health and explains coaching process and conducts interventions for Veterans
  • Conducts Whole Health Facilitated Groups and provides follow-up care through telephone clinics or secure messaging
  • Actively guides Veterans in the exploration of core elements of personal health planning
  • Develop treatment plans for Veterans focused on behavioral interventions
  • Reviews current health behaviors and develops customized treatment plans for Veterans
  • Utilizes motivational interviewing and health coaching techniques and processes
  • Uses VistA and CPRS to track and support patient progress
  • Conducts outreach activities to inform Veterans of available programs and provides education opportunities for staff in order to promote health coaching
  • Additional duties as required

Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Work Schedule: Monday – Friday, 8:00 am – 4:30 pm

Telework: Position is suitable for Telework and may be available to employee at full performance level.
